OOS appreciates our affiliate relationships!       

 Affiliate agrees to provide above services on above dates with said vehicle/s. Once job is accepted, if you cancel this job at a later date, you have automatically terminated your relationship with this company.  This job may not be subbed out for any reason or all payments will be forfeited.  Affiliate is responsible for verifying accuracy of all addresses, information & times 24-hours prior to day of service.  All drivers on charter must keep vehicle dedicated to client only and not perform other jobs when on charter.  If client needs the driver phone number for any reason, driver must give the phone number on a blank piece of paper and not the back of his business card.  Should any material, i.e. cards, brochures, paperwork, video, pictures, etc…be passed to our customer, or should our customer be asked for a credit card, credit card information, credit card slip obtained from them or any email or addresses, any services or other vehicles that your company offers or affiliates that you may know offer, cannot be discussed to further a future relationship with this client or all monies due from OOS  WILL BE FORFIETED WITHOUT QUESTION. Drivers to be on time, clean and appropriately dressed & vehicles clean inside and out.  One hour will be deducted for being late more than 5 mins. At no time will anyone from the affiliate company disclose the amount that their company charges or we paid affiliate for said job.  Drivers to call OOS upon arrival and have our driver log signed off on at nights end, if charter. Log to be faxed next AM.  There should be no information given out from your driver about our company at any time, or our relationship with your company.  No solicitation of our Wedding planners permitted. Any additional information that the customer may require or ask for, or any future transportation requests should be referred directly to our office with our phone number.  Please make sure your driver is equipped with our card!  All agreed amounts will be paid after job is performed AND, we have heard back from our customer about your service. You also agree to all terms in our contract with customer.

Theft of service actions are bound to Orange County, Florida court system & all fees pursuant to obtaining relief are the responsibility of the affiliate.The undersigned agrees to all times, dates, addresses & terms .
