OOS appreciates our affiliate relationships!
We as the affiliate agree to provide above services on above dates with said vehicle/s. Once your job is accepted, if you cancel this job, you have automatically terminated your relationship with this company. Affiliate is responsible for verifying accuracy of all addresses, information & times 24-hours prior to day of service. We will not card your customer. Our drivers will be on time, clean and appropriately dressed & vehicles clean inside and out. Any additional information that the customer may require or ask for or any future transportation requests will be referred directly to your office with your phone number. Once you have booked us for a job, if it is a no show or otherwise canceled you still owe OOS the entire amount of your booking. Payment is expected either on the day of pickup or within 10 days by mail or credit card on file. This form to be cut, pasted for use on all jobs. You & your client agree to all terms of our contract.
Theft of service actions are bound to Orange County, Florida court system & all fees pursuant to obtaining relief are the responsibility of the affiliate.
